Register With The Practice?

If you wish to register with the practice, one of our reception staff will be happy to arrange an appointment either with the treatment room nurse or with a doctor if you wish to consult with a doctor at the time of registration. You will be asked to bring your medical card or to complete a form if this is not available. You will be asked to fill in a short questionnaire on your past and current medical history. This is confidential and is used to help us to give you the best possible care until we receive your medical records.

You must reside within our practice area, which expands to a six-mile radius of the health centre.
